I acknowledge that if an additional Card(s) is issued on my Card to the person name above, I agree to be liable for all Charges to my Card account, and for those Charges incurred with any additional Card(s) issued. I agree to be bound by the terms printed on the back of the Card unless i notify the bank in writing and cut the Card in half, returning both halves to Fidelity Bank. If either the Fidelity Bank or | terminate the Card agreement, all statements and disclosures are correct and that all transactions are and will be beneficially owned by me. Further, I authorize Fidelity Bank where necessary to verify my source of funds by means of exchange of information about me now and in the future with other financial institutions, etc, and hereby agree to indemnify and save harmless Fidelity Bank, affiliates and subsidiaries from all Claims from such exchange.
